I. Fill in the required information in the form
II. Press ‘Print’ to take a print out of the form you filled
III. Sign the printed copy
IV. Send the scanned copy of the signed form to – auth@eastwesttravelinc.com

I agree, i will pay for this purchase and indemnify and hold eastwesttravelinc.com / EWT, Inc harmless against any liability pursuant to this authorization.
